Dr. Hickman will look for the two ends of the vas deferens and free them from the surrounding scar tissue. He will them perform a microsurgical repair of the vas using an operating microscope and 10-0 nylon suture, which is thinner than human hair. He will carefully align the lumen of the two ends of the vas and create a watertight closure. Before performing the closure, he will first take a sample fluid from the testicular end of the vas deferens. Since the testicles don’t stop producing sperm even after the vasectomy, the fluid should contain some sperm. It some cases, whole sperm may be found in the fluid, but it is more common to find sperm parts.

A third possibility is that the sample fluid doesn’t contain sperm or sperm parts. The third case is indicative of an epididymal blowout, which means there is a blockage either at the vasectomy site or there is a rupture of the vas in the epididymis. In this case, Dr. Hickman will have to perform a vasoepididymostomy in addition to the vasovasostomy.
